The make and model number for this bracket is unknown.
The bracket is 12 inches long. It is sealed at both ends and used as a “bracket” attached to the wall, but at floor level. This design allows the exhaust hose to fall to floor level, exhausting through this bracket and a pre-cut hole in the wall. The exhaust hole is just under 5 inches around. Note that one of the screw holes is incomplete; however, new holes can be drilled to fit the collar on your exhaust hose. Please refer to the photos to ensure that this is the bracket that you need.
